My name is Chris Walters and today is June 19th, 1997 and I am on a medication prescribed by Dr. Fleisher of Tyler, Texas known as Librax or Clindex as treatment as medical authorities explained for "Illitis & Collitis" or "spastic colon" and because of having up to 15 bowel movements a day cannot work/function. A brief history of current medical treatment is not intended to replace pending information on SSI applications forms or appendices:

1. Food Stamp worker in Tallahassee, Fla in April, 1997 Ms.Gillette stated Fla Children and Fa mily Services does not assist disabled clients with medical problems to get medical evaluation or treatment. Ms. Gillette insisted I register for work with Fla Employment Dept who sent me to interview with Cafe D Lorenzo who wanted to know why Chris Walters a USFDA registered carrier of Hepatitis C should work as a dishwasher in their kitchen?

2. Medical Treatment/Suggestion: Food stamp worker Hoyet Frier suggested if Chris Walters wanted something to eat he should "slap a police officer up side of the head as hard as possible.

3. Shelter social worker made referral to local health clinic where Dr. Williams suggested Chris Walters has Melanoma of neck need biopsy,stated she couldn't be bothered treatment tumor in groin causing disability. Attempted referral for medical work to Health Agency: deep in "Frenchtown" site of frequent murders and alleged drug activities. Declined to enter this area reasons personal safety.

4. Chris Walters walked to Birmingham where Health Care for Homeless Inc. Made excellent intake, screening, referal to Cooper Green Hospital:

a. Dr. Harris biopsy Melanoma on neck, place Sivler Nitrate on wound.Schedule for surgery on neck. Biopsy wound got infected, Why?

b. Dr. Harris states he is afraid to operate on disabiling tumor in groin because of location, hair in area, danger of infection....

c. Hepatitis C test.

d. Health Care Officials make arrangments with social worker Mark Rowe for mandatory housing who argeed however, neglected to leave his instruction wirtten in "book" for weekend employees.

e. Weekend employees tell Chris Walters you can't remain here no written instructions, Chris Walters gives explanation letter to Dr. Harris/Dr.Johnson leave Cooper Green.

5. Chris Walters walks to Meridian, MS where Community Action Agency/ Shelter:

a. Makes two long distance calls to Dr. Johnson where Chris Walters explains lack of housing to secretary, and faxes request for medical information which I am told was not sent.

b. Makes referral to local health clinic where Nurse McCarty tries to get all medical records faxed from Birmingham.She states records only partial and tries to schedule surgery in Jackson, MS after having been told shelter operators in this city do not recommend such actions because shelter area controlled by gangs.

6. Chris Walters walks to Longview, Tx where Good Shepherd Medical Center and Dr. Foster diagnosis Melanoma on neck and makes referral to Dr.Larry Smith for surgery. Dr. Foster has social worker attempt to secure housing committment from Salvation Army:

a. Social worker at Salvation Army declines 1 day extension in lodge with written request from Doctor as does not merit respect until after surgery.

b. Local Methodist Minister Jerry Turner states Salvation Army often is problem and arranges long distance phone call to Tyler, Texas to Salvation Army which agrees to 7 days housing.

7. Chris Walters walks to Tyler, Texas is approved for Salvation Army Housing, Smith County Indigent Health Care, and Dr. Steph does surgery on Melanoma on neck, and removes tumor in groin. Chris Walters can bend over, stoop, and do various activities again. Dr. Steph referrs Chris Walters to Dr. Fleisher for long term bowel problems who schedules colon exam and puts Chris Walters on medication Clindex Capsule which causes bowel movements. Salvation Army manager states Chris Walters cannot continue to extend in housing to continue in treatment unless he agrees to provide free labor to work for Salvation Army for free. Type of work requested: working on truck, and in store moving furniture...

8. Chris Walters walks to Longview, TX is nobody will approve temporary housing or assistance to continue bowel exam or work with Dr. Fleisher in next city.

9. Chris Walters walks to Ruston,Louisiana and Fire Department makes written request for assistance to Police Dept for housing...... Local Salvation Army asks police to dump Chris Walters on highway in Quachita Parish.

10. Chris Walters walks to Vicksburg, where River City Rescue Mission states they want Chris Walters to do free work as volunteer to stay. Dr. Robbins at Hospital prescribes 2 medications which he also states no agency in this community will fill for indigent person.

11. Chris Walters walks to Meridian, MS where local social and medical authorities disapprove long term medical treatment plan to:

a. Locate long term housing-2 agencies couldn't be bothered to make calls.No clothing, other request denied.

12. Nurse McCarty states she cannot get medical records from either Tyler or Birmingham by fax and redoes Hepatitis C-I do not have this.

a. Attorney Kathering Blackwell request Reverend Hubbard at shelter allow Chris Walters to stay until next Thursday to complete evaluation of SSI.Reverend Hubbard agrees.

b. David shelter manager explains Chris must leave shelter if he tries to use bathroom in other men's dorm. My dorm bathroom broken for at least 3 weeks.

13. Chris Walters walks to Tuscaloosa, Ala seeking assistance from Ms. Barbara Smith Community Service Program of West Alabama, Inc. (205-758-4756) who is trying to arrange:

a. Long term housing with Pastor Rick Hagan at His Place, Opelika, Al who appears to be dodging Ms. Smith calls.

b. Ms. Smith spoke to Birmingham Health Care (205-323-5311) who according to Sharm Brammer will not provide further Health Care Chris Walters. Reason: Birmingham Health Care states Chris Walters missed schedules surgery without giving any notice.

Question of Further Treatment Based on Actions of
Medical and Social Authorities Evasive/Deceptive/Abusive

1. Letter to Dr. Harris/ Johnson concerning loss of housing arrangement is attached, records of phone calls from Community Action Agency 2 phone calls to Dr. Harris office can be subponea, with fax request from Nurse McCarty to Birmingham who she states refused to provide complete records,contradicts current statement of Health Care Agency that Chris Walters missed schedules surgery without giving anyone Notice.

2. Nurse McCarty seems reluctant to provide copies to Chris Walters of medical records and declined to sign recommendations for action. You can't sign it, I can't accept it.

3. Diagnosis/Treatment of medical problem discontinued on several occassions by homeless shelters (Title 42 USC 11301) who state they wish Chris Walters to discontinue treatment to work for them for free. Under existing laws of Wage and Hour Administration Chris Walters is legally entitled to be paid for all work done.

4. Dr. Fleisher wants colon exam done while Chris Walters is unconsciou Why? Chris Walters has already has procedure in 1964 as dependent in USAF and is aware it does not require such medical treatment. Does Dr. Fleisher plan to remove polpys, disease tissue, and doesn't want Chris Walters to be aware of findings?

5. Dr. Steph removed tumor from Chris Walters groin and at Chris Walters request cut it open in office. Lab report of infected cyst...or process does not match tumor interior with healthy pink, solid issue, encapsulated with attached blood supply. Lab report goof?

1. When medical authorities referr Chris Walters to diagnosis/treatment to area controlled by gangs, drug activity, where persons of common sense don't go what option does Chris Walters have but discontinue action based on personal safety?

2. How is Florida State doing on reply to inquiry made to US Senate Judiciary Committee about therapy to "slap police officer" or send plague carrier to work in restaurant?

Is Disability Being Maintained For Profit

For years various medical authorities declined to complete diagnosis/ treatment of tumor in groin which made it impossible for me to bend, stoop, lift, and preform various other activities. It took Dr. Steph a whole 10 minutes to excise the tumor and I have regained use of large part of my body. The nonproits reported they posted $147 Billion Dollar yearly profit and I will suggest to SocialSecurity Officials that actual cost of my being homeless to society is about $15,000 per year including all the hidden costs. The nonprofits/shelters obviously believe Chris Walters is a vested financial interest and have impeded medical treatment and:

1. Arranged "criminal history" on NCIC to disable Chris Walters bondability.

a. Secured federal Court Order allowing arrest Chris Walters if he attempts to get job by using nonprofit phone of mail.

b. Federal Court Order permits some agencies to steal US Mail.

A primary motivation for Chris Walters to apply for disability for SSI at this time is that getting on disability:

1. Total control of Chris Walters life through housing, food, clothing,...ends because Chris Walters can get apartment....

2. Chris Walters can proceed with possible treatment without further interference.

3. Chris Walters can make strong showing for possible vocational rehabilitation.

1. Disabled from 1990-1997 by tumor in groin which made it impossible to bend, stoop, lift, or work which was surgically removed by Dr. Steph in Tyler, Texas in May, 1997.

2. Disabled from 1972-current by long term bowel problems which includes frequent need to use bathroom and requires additional diagnosis/treatment.

a. Question to Mr. Fred Avitt and Robert Amos friends of 10 years if Chris Walters actually plugs their toliet and does Chris Walters odor offensive?

b. Question to Willie, employee of South Harbor Shelter if Chris Walters odor so offensive have to wash clothes daily and give special hygiene instructions.

c. What employier would hire a person with this problem?

3. Maybe disabled from more extensive "vocational rehabilitation" by Dr. Steph instructions to avoid more extensive exposure to sunlight and threat of more Cancer.

4. Question of Bondability/Employability: Normal types of work might include security guard, bank clerk, or other clerk all require bondability which may not be possible and is in issue in USDC and before US Senate Judiciary Committee:

a. Question: How can a person who has never been tried in criminal proceeding have an NCIC "criminal history".

b. Question: Have persons associated with Chris Walters "vocational project" actually been harassed by members of United States Goverment and are they are risk?

a. Shaving: Have slight problem shaving because of raised area after surgery in throat.

q. Bathing: After surgery on groin can bend down and wash better because tumor gone.

a. Dressing: After surgery on groin can bend down and reach toes again, easier to tie shoes.

b. Bowel problem since 1964: real problem in hygiene and smell department. If not having regular bowel movements other persons complain of odor and often recieve little gifts of soap, deodorant, and other essential items. Willie (601-483-0776) Shelter employee at Meridian often asked permission to wash my clothes and gave generous hygiene items. Sorry Willie, smell is inside it won't wash off.

c. How can a person who has 15 bowel movements in a day Work for a firm?

d. What company will hire a person with permanent bad smell, possibly caused by chronic bowel problems?

e. Urine Drip: Have chronic urine drip into drawers, is this normal or common?

f. Agency Question for "Willie" employee of shelter does Chris Walters smell so bad that you requested he wash his clothing daily in June, 1997 and gave special hygiene instructions:

q. Has there been any change in your ability to do chores around the house,such as shows to the right due to your illness or injury?

q. Because of successful surgery in groin can again do yard work, light cleaning, dishes, and other minor functions.

a. Bowel problem since 1964, recent disability change due to medication May, 1997; not detriment as long as bathroom open for quick emergency dashes up to 15 times a day.
q. Has there been any change in your ability too shop or do errands such as shown to the right, due to your illness or injury?

a. Buying groceries: can again life heavier objects due to results of surgery in groin.

b. Bowel problem since 1964 and with medication since May, 1997 limits ability preform some errands because of need be near bathroom.

q. Has there been any change in your ability to prepare meals, eat, such as show to the right, due to your illness or injury?

a. Don't cook.

b. What eat has a lot to do with bowel problem. Tummy likes vinegar, pickles, mustard, and similar acid items. Foods such as fried chicken cause near fatal shut down of bowels. (I love chicken by the way). Dr. Fleisher has no explanation of how certain foods could be involved but states eating pickles couldn't hurt.

c. Throat injury stemming from strangulation attempt by crazy person on grounds of Texas Capitol in Austin, TX about 1990.

d. Teeth: several cavities and holes require dental repair.

q. Has there been any change in your ability to handle your money responsibilities, such as shown to the right, due to your illness or injuries?

a. No job, no money, no bills, no problem, homeless for about 10 years, unable to work because of diabilities.

q. Has there been any change in your ability to get around such as shown to the right due to your illness or injuries?

a. Driving: near sighted and blind at night. If I had license I would not recommend you sit in car I would drive.

b. Using Public Trans: unfortunate bowel accidents can occur to those who stay from bathroom.

Other: Disability since May, 1997 Walking/Hitchiking: Takelots of napkins. Have mastered art of leaning on tree to avoid misfortunes in drawers. Eastern US has lots of trees, any of who I know on first name basis. Western US-oh well, hope your blind.

Dr. Steph asked me to stay out of sun because of increased risk to Cancer and suggested no trips to Arizona this summer as a disability or limiting matter since May, 1997.

q. Has there been any change in how you spend your time doing things, such as shown to the right, due to your illness or injury?

a. Recreational: nothing severly physical since 1972.
q. Has there been any change in your ability to do the activities on the right due to your illness or injury?

a. Sitting: Disability caused by tumorin groin since 1990.

b. After surgery can sit better without tumor in groin

c. Sitting: Disability caused by tumor in groin since 1992. Since 1990: Yeast infection on posterior chronic problem.

d. Standing: Disability caused by tumor in groin. Can stand longer after surgery, removal tumor.

3. Walking: Legs work better than average, can walk almost for ever. Only problem recurrent sores on feet need doctor examine for infection/diabetes.

4. Kneeling: Disability caused by tumor in groin since 1990.Can kneel again after removal tumor.

5. Squatting: Disability caused by tumor in groin since 1990. Can squat again after removal tumor.

6. Climbing: No change, don't attempt.

7. Bending: Disability caused by tumor in groin since 1990. Can bend again after removal tumor.

8. Lifting: Disability caused by tumor in groin since 1990. Can life objects over 10 pounds after removal tumor.

9. Reaching: Disability caused by tumor in groin since 1990. Can reach in downward direction again after removal of tumor.

10. Using Your Hands: Since May, 1997, apparent Cancer on hands not impediment at present time. More use hands than before, vocational training.

17. Swallowing: Since 1990, problem due to unknown injury in throat. Bowel movements: due to medication frequent at this time.

a. Cafe De Lorenzo: April, 1997 says you send another Hepatitis C carrier to work in their kitchen they will plant the responsible party in pasta.

b. National Bank Commerce: 1981-1983) San Antonio Texas spiritual home of all Ceder trees. If you have "cedar fever" this is not good place to live.

c. Manpower, Salisbury, NC 1993: Not ceder tree in sight? Let me understand this, we live in Piedmont and air pollution is major health problem-bye.

Special Environmental Risks/Factors
Unique to Homeless (Title 42 USC 11301 SMHAA)


1. Exposure Radiation: most shelters put clients out into weather at 6AM each day and in many areas of western US this can cause prolonged exposure to sun resulting in Cancer (Melanoma for instance.

2. Exposure Incliment Weahter: most shelters put clients out in weather at 6AM 365 days a years and this can in cold weather cause hypothermia or hot weather heat exposure (Hospitalized Casa Grande, AZ 1994). Either can condition can be fatal.

3. Hygiene: Many shelters lack adequate hygiene items ranging from means to clean clothes, to inability to provide clean clothing, lack of soap,mouth wash, tooth brush & paste. Personal hygiene for homeless is a major area of concern. Hygiene lacking can cause skin disorders.

4. Diet: Most shelters feed meals which are high in starch, sugar, and general fat content. Clients can become over weight, or develop diabetes. Diet in general would not support work type activity unless supplemented by food stamp items such as juices, fruits....

5. General Exhaustion: Many shelters provide clients only 2 or 3 days housing per month and clients may have to walk/ hitchike up to a hundred miles at least twice week to receive more service because non profits are operating homeless travel agency.

Medical Wish List For Chris Walters


1. Regular supply wintergreen rubbing alcohol.

2. Colon test/evaluation of bowel problem.

3 Unfilled prescriptions 1 for year on posterior to be filled. Two Nurse McCarty at Meridian simply stapled in my file.

4. Examination of throat; Dr have tool look down throat?

5. Evaluation probable "Melanoma" on right hand (not walking 1200 miles for surgery)

6. Kidney dye test; kidney function.

7. Test for glasses (near sighted)

8. Teeth: dental abcessed teeth health problem.

9. Dietary prescription: pratical for bowel problems.

10. Evaluation skin disorders: test blood sugar for diabetes.

11. Written medical directive: stay out of sun.

12. Medical authorities suggest; idea for allergies.

13. Food Stamp application for July, transfere unused benefits from Texas to Tuscaloosa.

Dear Ms. Shuttleworth:

I am requesting my claim for social security supplemental income be moved to Tallahassee, Florida for following reasons:

1. Offer by Barbara Smith of Community Service Program of West Alabama at 601 17th Street, to recieve mail and messages for my case was denied or overruled by agency Director and your agency and District Attorney have recieved my Complaint to Civil Rights of US Dept of HHS.

2. Salvation Army indicated I have no housing options or place to stay under our contract after Monday, July 7th, 1997.

3. No reply was recieved sent 3 local doctors requesting medical treatment.

4. Civil Rights Complaint mailed to US Dept of HHS about previous problems in Florida should defer any more request by Fla Dept of Children and Family Services to have disabled persons or food stamp recipients "slap a police officer" if they want something to eat.

I can't thank your office enough for assisting my efforts to file a claim for Social Security Supplemental Income and answering many questions on how to complete forms.

*Has Chris Walters ever clogged your toliet? Does Chris Walters have frequent odor and causes you to ask him to sit across room when visiting?

Clubs, social groups, church: Got good bathroom and unable to smell?

Other: It is kind of embarassing to have to scratch in public but that yeast infection is permanent on posterior and itches as disability since 1990. Dr. Robbins suggested my treatment of peroxide was giving the yeast something to eat. It took someone years to explain this to me?
