I certainly appreciate your coming to our office several days ago and being interviewed.
As you can see from the enclosed copy, we have written to the Social Security Administration to join in your claim for benefits. It usually takes the Social Security Administration 3-5 months to process the initial application. If that
is denied, we will recive a reconsideration decision. If you claimis denied on reconsideration, we
will request a hearing before an Administrative Law Judge. It generally takes the Social Security
Administration 10-14 months to schedule your hearing.
When you recieve a decision on your claim, wait 3 business days, then call our office and verify that we have
recieved our copy of the decision.
In the meantime, we will obtain medical information we think is important and helpful to your case. As soon as I have
heard anything further from the Social Security Administration, I will be back in touch with you.

FEE AGREEMENT

KNOW ALL MEN BY THESE PRESENTS, That:
I hereby exclusively appoint and employ CHARLES TYLER CLARK,P.C. a professional
corporation, Birmingham, Alabama, including only the attorneys of that firm as
follows; CHARLES TYLER CLARK, WILLIAM M HANLIN, DARRYL W. HUNT, FELICE ANN S,
GOLDSTEIN, AND MARILYN H MACEY to prosecute for me on my behalf and on behalf
of my heirs, executors, administrators, and assigns, all claims for benefits
as provided by the Social Security Act, with full right to represent me at all
administrative levels.
We agree that if the Social Security Administration favorably decides such claim,
I the claimant, will pay my represenative a fee equal to the lesser of 25% of the
past-due Social Security and SSI benefits resultingfrom my claims for benefits as
provided by the Social Security Act, including 25% of any auxiliary benefits due
my spouse and children, or the sum of $4,000.00
I understand that I will be responsible for the costs of medical examinations,
reports, and records, as well as reproductions, postal service and long distance
telephone costs. I understand that these costs are separate fromthe
attorney's fee and must be repaid regardless of the outcome of the claim(s).
If I am paid a lump-sum check for past-due SSI benefits, I agree to bring that
check to the said attorney's office, so the correct amount for the attorney's
fee, along with any costs, can be deducted and the attorney's fee put into a
trust accoung pending a determination of the final fee.
In the event that my attorney is unsuccessful in obtaining benefits for me, I do not
owe him any attorney's fee and I will be responsible only for the costs.
I hereby acknowledge reciept of a copy of this fee agreement.
