Chief Complaint
Chief Complaint: Partially excised lesion of right inguinal canal with path report of squamous call
carcinoma in situ. Several other lesions in left goin and inncer thigh as well as on rectum. Wound
desiscence.
History of Present Illness
History of Present Illness: The onset of this illness was in January and February, when I noted irritationof inguinal area
(biliterally) by my underwear, Upon investigation, there werenoted to be "growths" in my groin. These were presented to
my family physician Donna Becker, D.O., who referred me to Dr.Patricia Gentile in here Aransas Pass office. The date of
my referral to Dr. Gentile was Feburary 7, 1990. I was seen in Dr. Gentile's office subsequently and
examined. She recommended surgery for removal of said lesions. This was first delayed due to the death of my
mother-in-law in San Antonio, TX. I was next seen by Dr. Gentile on Feburary 21, 1991, whereupon, two lesions
were removed surgically in here office. Speciments were sent to National Health Labs in San Antonio, Texas.
The wound dehisced, as she predicted, due to the use of 6-0 catgut sutures. I was seen in here office for
post-op followup on Feb 26 and Feb 28th, On the last visit, Dr. Gentileadvised me that she had called the lab and
received the report of squamous cell carcinoma in situ**. Upon hearing this news, I requested Dr. Becker
to refer me to an onocologist and I saw Dr. Kirby Barker in Corpus Christi on March 6, 1991. Dr Gentile was advised of
this request and my wish for a second opinion as to other or additional modalities of treatment. Dr. Barker recommended
completion of surgery with removal of the remaining lesions and no other treatment such asradiation or chemotherapy.
He also called Dr. Becker and Dr.Gentile to advise them of the physical findings and recommendations.
I attempted to reach Dr. Gentile by phone, leaving a message in her Aransas Pass Office on March 7 and 8th
succussively. To date I have received no word or call from her. As I wishto have these lesions removed as soon
as possible, I am seeking your help in this matter.
Past Medical & Surgical History
1. Thyroidectomy in 1956 at Lackland A.F.B. Texas for DX of benign adenoma.No sequella.
2. Hysterectomy at Homestead A.F.B. in 1968 for uterine rolapse, cyocele, and rectocele.
An A&P repair was also done at this time. The nature of this surgery was vaginal.
3. Fistulectomy following and I&D of the rectal abcess t Mather General Hospital,
Port Jefferson, N.Y. 1977. Excession of lesions of the inguinal canal lesions as described in HPI.
4. Medical illness include HNP of L5-SI disc in 1973 treated with pelvic traction and hospitalization
for pending laminectomy. This surgery was canceled due to pneumonia and subsequent DX of pulmonary tuberculosis.
5. A C&S of the sputum was found to be positive for AFB. In retrospect, a
PPD was positive about 6 months prior to this occurence and was treated with INC and Pyridoxine for this
period of time by Miami VA Hospital, where I was employied at thie time. Treatment for the Tuberculosis
was underway with isolation and administraton of Rifampin and Ethanbutol, there was no evidence of this
DX on X-Ray. Three sputums were submitted of the Florida State Board of Health and all returned negative.
The medications were then DC'd.
6. My main problem is that of allergies with asthmatic bronchitis. My medical needs have been provided by
Humana Health care of San Antonio, TX for the past few years under this staff physician, Dr. John Wood.
Since I moved to Rockport in 1989, I have had to change my health plan to Costal Bend Health Plan as Humana
does not cover this county.
7. Donna Becker, D.O. Of Aransas Pass has been my family doctor since Feburary 1990. I have high cholesterol and
triglycerides on blood chemistries since about 1984. The last reading was a cholesterol of 305 and high Triglycerides.
Social HX
Have traveled extensively with A.F. husband to the orientand throughout the U.S.
Married X3, have had 3 sons, who are living. Habits include smoking 1 1/2 packs
per dayX45 years, drinking socially, unsually happy hour varietyand drugs as prescribed
by physician. My occuption was that of medical transcriptionist for 25 years. Now retired from Audie
Murphy VA Hospital of San Antonio, Texas.
Present Medications
1. Mevacor 20 mg 1 tab q.d.
2. HIsmanal 10 mg. 1 tab q.d
3. Slo-Bid 200 mg.
4. Gltovsp 1 tab t.i.d. taken p,r.n)
5. Premarin 0 625 mg 1 tab q.d.-5 days on and off rotation
per Dr. Becker.
6. Additional X included OCUSORT lip pads t.i.d
7. Tobrex-ophthalmological oingment.
At this juncture, I am very distresed both phsically and mentally as to the treatment and prognosis
of the SCCA and existing lesions still in situ. I hope these histories ect. will expedite action of
