During discussions with these folks and another skilled nursing facility I provided them with extensive medical records and documentation and what my case plan expectations are.

The Activities Director Andrianne; Rehab Director Eric; Social Worker Director have requested I prepare a discharge plan for departure and brought in an outside consultant on 1/9/2019 to discuss my options
