I have already delivered a copy of my advanced Medical Directive that I will not participate in sleep study as I already have sleep study and there is no reason to pay $5700 to receive duplicate results

Between my $633.00 a month and thousands of dollars in state payment the new management actually turned 3 meals a day into 2 meals and a snack while spending a lot of money to remodeling the facility. I can actually eat better in the average homeless shelter and have less friction from the other residents than I do in this assisted living facility

In the course of ongoing unsolicited conservations John Fravel explains he has to steal a lock from the other building so as to prevent the staff from enter.

1. John explains they have the right to search the room at anytime without his permission for drugs or other illicit substances

2. John explains he is in debt to a Bishop of the LDS Church for several hundred dollars he cannot repay

3. John is sleeping almost 20 hours a day part of the month and seems to go without sleep for days at other times and his behavior and conversation suggest possible substance abuse

4. John explains he is supposed to be on recover from drinking however is apparently not attending AA and according to him does not have a sponsor to turn to

5. John explains he has apparently run 3 or 4 previous residents out of the room. He is in habit of getting in bed with his wife and finds sex challenging with another person living in the same room

I spoke to new Executive Director Katherine Alderman and employee Jordan about problems I am having with Resident John Fravel. I asked to be moved to another room. The management explained they would reply to my concern and proposed move within 2 days-quite a while later I haven't heard from them.

Withdrawl from drugs must be tough. My room mate sleeps maybe 20 hours a day and only gets up around meals-screams at staff when they knock on the door to see if he needs anything. It appears some of the staff didnt take a shine to being screamed at and have quit.

The staff has washed my clothes and the onduty staff Kerah is refusing to return my laundry. I have to ask to call the police to get my clothes back. The staff then decide to return my clothing without any further problem

The Ombudsman for this Region came out and discussed the problems I am having with John Fravel and I suggested she speak to him and see if someone could get him a social worker or other professional to address his concerns

An elderly Resident Connie goes missing without explanation in the middle of the night during a blazing snow storm and has no way to exist locked building. Her belonging are still in room and no one has an apparent clue as to what has occured. Connie was at dinner several nights before explaining residents were trying to rape her and she was crying that they were going to kill her

1. John Fravel has been brief by the Ombudsman and explained he cannot have anyone living in his room that he cannot not trust to snitch him out about 6:30 PM 11-29-2016. John has apparently chosen to communicate his intent after being advised by the Ombudsman not to contact Chris Walters with any futher complaints

(a) (1) Whoever kills or attempts to kill another person with intent to retaliate against any person for— (A) the attendance of a witness or party at an official proceeding, or any testimony given or any record, document, or other object produced by a witness in an official proceeding; or

(B) providing to a law enforcement officer any information relating to the commission or possible commission of a Federal offense or a violation of conditions of probation, supervised release, parole, or release pending judicial proceedings, shall be punished as provided in paragraph (2).

(2) The punishment for an offense under this subsection is— (A) in the case of a killing, the punishment provided in sections 1111 and 1112; and

(B) in the case of an attempt, imprisonment for not more than 30 years.

(1) the attendance of a witness or party at an official proceeding, or any testimony given or any record, document, or other object produced by a witness in an official proceeding; or

(2) any information relating to the commission or possible commission of a Federal offense or a violation of conditions of probation, supervised release, parole, or release pending judicial proceedings given by a person to a law enforcement officer; or attempts to do so, shall be fined under this title or imprisoned not more than 20 years, or both.

(c) If the retaliation occurred because of attendance at or testimony in a criminal case, the maximum term of imprisonment which may be imposed for the offense under this section shall be the higher of that otherwise provided by law or the maximum term that could have been imposed for any offense charged in such case.

(d) There is extraterritorial Federal jurisdiction over an offense under this section.

(e) Whoever knowingly, with the intent to retaliate, takes any action harmful to any person, including interference with the lawful employment or livelihood of any person, for providing to a law enforcement officer any truthful information relating to the commission or possible commission of any Federal offense, shall be fined under this title or imprisoned not more than 10 years, or both.

(f) Whoever conspires to commit any offense under this section shall be subject to the same penalties as those prescribed for the offense the commission of which was the object of the conspiracy.

(g) A prosecution under this section may be brought in the district in which the official proceeding (whether pending, about to be instituted, or completed) was intended to be affected, or in which the conduct constituting the alleged offense occurred.

John Fravel explains he found bed bug on or near my bed. Exactly what John is doing in or near my bed remains a great mystery

It is believed that this medical work could and should have been done with in 4 or 5 months without any problem

Shannon Smith the case Manager met with me 12-5-2015 and suggested I stay for extended period of time and see if I can get into assisted living. So we will see what can be done throught Janurary 1,2016

The Church Of Jesus Christ puts out a lot of time effort and resources witnessing for Jesus. Between 2 year missionary trips; home visits; and other activities a person would have to wonder why the folks in this region are unwilling to assist persons with medical problems?

The patient complaint about severe long term pain in lower left kidney and bleeding through back of both hands. The hospital staff did a bunch of test except of course scan of kidney region to look for blood clots; bleeding into kidney or other problems.

Strange I thought medical professionals were supposed to be healers who resolved medical problems instead of dragging them out for years and years to increase profits 10 Doctors 5 years to do minor surgery for cancer in office. 45 Doctors took 15 years to do colon exam. 160 Doctors treated chronic blood clots and 4-5 hospitalizations a year at $400,000 until 1 doctor prescribed Xarelto-a pill a day..131 Doctors Treat Chronic Infection 1 doctor solves problem with hospital visit.
