Greensboro, NC is in the heart of the North Carolina "Triad". A former industrial area known for furniture and textile production the region is home to 3 major medical teaching hospitals. With a large supporting network of homeless shelters; medical resources; and interconnecting transit systems the area should be ideal to assist my medical work and placement in an assisted living facility.

My long time friend Natalia Tuchina invited me to the Golden Gate Russian traditional and folk presentation. While I did not make the presentation it looks like lots of fun.

As the enclosed materials indicate I have asked several Congressional Committees to review aspects of my SSI claim as it relates to 3rd party tampering primarily by various concerns associated with the US Department of HUD.

When a nonprofit agency indicates they do no accommodate persons with disabilities or ends a course of medical treatment being paid for by Medicaid the medical payment system is losing an estimated $50,000 per event.

As your office recalls the several federal courts have upheld the rights of the Commissioner to a hearing pursuant to Title 42 USC 405(g)(H) Judicial Review. I am unaware of any 3rd party right interplead; join, or otherwise litigate in the matters.

I had recently moved to Greensboro, NC and put in a change of address and the staff of the local office had responded but not update the address nor put in for Medicaid as you can see from the attachment.

Because of what appears to be extensive ongoing 3rd party tampering I have forwarded the materials on CD-Rom (Exhibit C) to the US House Committee on Judiciary. The focus of the 300 megabyte report is to review how 3rd party tampering causes enormous delays and increases costs of the SSA process.

Plaintiffs Exhibit C has had a major reformat to make it easier to read and prepare for the Commissioners eventual hearing under Title 42 USC 405(g)(H) Judicial Review. Medical Citation Janurary, 2011: Chris Walters MRN DOB 10-17-1950 SSI 261-90-6357 was hospitalized at Wake Forest Medical Center in Winston Salem, NC for 4 days and treated by Dr. David Miller for Deep Vein Thrombosis using daily dosage of 10 mg of Coumadin. Chris Walters complaints of pain in right kidney were not addressed when the patient was discharges.

*** Advanced Medical Directive will not let doctor balance Coumadin on outpatient basis. If it can't be completed in hospital it won't be done at home.

When the patient began bleeding in the kidney a few days later the follow up treatment had to be done at Moses Cone Hospital at considerable costs to Medicaid.

1. The Wake Forest University Medical Center has been making referrals for Chris Walters to be seen at the Downtown Baptist Medical Center since before 2000 and in the 11 years the Downtown Baptist Medical Center has never made a referral for Chris Walters to be seen by a primary care physician for follow up.

2. The purpose of the fraud against Medicaid appears to be to create additional charges against Medicaid which will not resolve any medical issues in 1 visit.

1. In a 6 month period of 2011 3 hospitals could not balance and maintain a simple treatment of Coumadin for blood clots. All the hospitals duplicated expensive sonar exams rather than simply call for a copy of last hospitals findings Moses Cone performed a CT Scan and has no definite idea of why patient bleeding in kidney Moses Cone agreed to balance patient blood thinner in hospital and instead tried to swap more expensive treatment of surgical procedure to increase cost to medicaid.

*** Advanced Medical Directive Medical Citation January, 2011: Chris Walters was seen in the ER at Moses Cone Hospital in Greens by Dr. Fuller who noted blood in urine and advised patient to discontinue Coumadin. Dr. Fuller referral could not be completed because patient lacked medical insurance at this time.

The doctor ordered a new sonar exam of the patients legs and the Xray technician asked the obvious question as to why the doctor is repeating an Xray exam recently completed at Wake Forest Hospital instead of simply asking the other hospital to fax the results to Dr. Fuller

*** Advanced Medical Directive The patient Chris Walters moved to Burlington, NC and was seen at the emergency room where he was diagnosed with venous insufficiency and no evidence was seen of blood clots. The use of CT-Scan was a major financial burden on Medicaid which showed no clear results.

The Patient Chris Walters was admitted to Moses Cone Hospital in Greensboro, NC on March 2, 2011 with agreement of admitting physician for administration of 10 mg of Coumadin in hospital treatment for several days to achieve therapeutic range. The person reading this report can probably appreciate the patients surprise to have a technician appear in his room at 10 AM the next day and explain he was there to prepare the patient for surgery.

Patient "Who authorized surgery? Technician "Your Doctor did"

Patient: "Who is my doctor" Technician: "Doctor so and so"

Patient: "Tell doctor so and so the procedure is canceled"

Noon Time: Nursing Case Manager: "Hello I understand you canceled the surgical procedure� Patient: �Who is doctor so and so?� Nurse: Your Doctor:

Patient "Why isnt the doctor so and so discussing my case with me. Nurse: "Doctor so and so can't be bothered but did explained you will be discharged from hospital. Patient: "Why bother-sign me out I will leave-bye"

If any of my doctors had bothered to visit my medical records history website they would have noticed the recommendation by several other doctors for the patient�s chronic problems to be addressed in an assisted living facility for a while to successfully address difficult treatment issues. The expert/teaching doctor at Wake Forest explained the veins in legs are very tiny and stretched out and not able to support blood flow; so why would the patient want to have a venous filter installed which if you could get into a small vein which would further restrict blood flow and possibly cause more clotting?

The patient might suggest using a procedure apparently using with success in arteries. Cather in both legs and introduce of liquid helium which freezes obstruction which separates from vein wall-and obstruction is pulled out through Cather.

If you�re a doctor and can�t give a patient a 10 mg pill of Coumadin; check blood levels for several days and address possible minor bleeding through colon or kidney how much confidence would you have in the medical staff? I have also noted medical staff questioning where patient lives which is a problem related to US Department of HUD programs where neither the patient nor hospital have any legal standing.

We have an online audit submitted to Congress which shows extensive treatment by medical providers who clearly understand Deep Vein Thrombosis or blood clots are a potentially fatal illness and yet manage to fail to hospitalize the patient; administer some blood thinner and completing the medical treatment.

Examination of this link shows medical providers have an extensive history of making frivolous medical referral where the patient who has Medicaid is referred to a doctor who does not accept Medicaid:

The PA has prescribed 30 day supply of Lovenox at costs of 41800.00 which is no longer authorized by manufacurer *** Advanced Medical Directive This medical group has take over medical treatment 3/15/2011.

I am on the phone explaining I am having serious problems breathing; bleeding internally, and having other possible side effects to the Lovenox. The medical assistant explains she hopes everything works out ok and wanted to tell me the Doctor wants me to take new medication for high cholesterol?.

As the enclosed materials indicate I have asked several Congressional Committees to review aspects of my SSI claim as it relates to 3rd party tampering primarily by various concerns associated with the US Department of HUD.

When a nonprofit agency indicates they do no accommodate persons with disabilities or ends a course of medical treatment being paid for by Medicaid the medical payment system is losing an estimated $50,000 per event.

As your office recalls the several federal courts have upheld the rights of the Commissioner to a hearing pursuant to Title 42 USC 405(g)(H) Judicial Review. I am unaware of any 3rd party right interplead; join, or otherwise litigate in the matters.

*** Advanced Medical Directive Several trips to the ER at the Hospital showed the hospital would do ultrasounds but would not provide comprehensive treatment for DVT such as Coumadin. When I had a clot in superficial vein the hospital prescribed antibiotics.

*** Advanced Medical Directive Dr. Bryan Tepper explained no acute blood clot existed and declined to begin Coumandin therapy Dr. Terrance Kolb explains no evidence of any blood clot exist.

I was very happy to receive services from Prime Care and their referrals to other hospitals for medical treatment and assisted living facility.

Preparing operational reports costs $210.00

1. Recommended Investigation by Federal Grand Jury Dr. Long made many attempts to convince me to have surgical filter to prevent movement of blood clots and when I declined he had me held against my will and explained he would give me surgery wheather I wanted it or not.

2. The Winston Salem Police Department compelled Dr. Long to release me from hospital and told him he could not compel surgery.

3. The hospital staff held me for another hour or so so make sure their staff psychiatrist could interview me who declined to hold me for surgery against my will

4. The apparent purpose of the fraud was to provide an Intern named Lonnie an opportunity to complete a medical procedure he needed to graduate.

5. Hospital Patient Rights Hospital clearly recognises the right of patients to refuse service if they so desire.

Title 18 Sec 1209 Kidknapping (a) Whoever unlawfully seizes, confines, inveigles, decoys, kidnaps, abducts, or carries away and holds for ransom or reward or otherwise any person, except in the case of a minor by the parent thereof, when�

(1) the person is willfully transported in interstate or foreign commerce, regardless of whether the person was alive when transported across a State boundary, or the offender travels in interstate or foreign commerce or uses the mail or any means, facility, or instrumentality of interstate or foreign commerce in committing or in furtherance of the commission of the offense;

(2) any such act against the person is done within the special maritime and territorial jurisdiction of the United States;

(3) any such act against the person is done within the special aircraft jurisdiction of the United States as defined in section 46501 of title 49

(4) the person is a foreign official, an internationally protected person, or an official guest as those terms are defined in section 1116(b) of this title ; or

(5) the person is among those officers and employees described in section 1114 of this title and any such act against the person is done while the person is engaged in, or on account of, the performance of official duties, shall be punished by imprisonment for any term of years or for life and, if the death of any person results, shall be punished by death or life imprisonment.

(b) With respect to subsection (a)(1), above, the failure to release the victim within twenty-four hours after he shall have been u nlawfully seized, confined, inveigled, decoyed, kidnapped, abducted, or carried away shall create a rebuttable presumption that such person has been transported in interstate or foreign commerce. Notwithstanding the preceding sentence, the fact that the presumption under this section has not yet taken effect does not preclude a Federal investigation of a possible violation of this section before the 24-hour period has ended.

(c) If two or more persons conspire to violate this section and one or more of such persons do any overt act to effect the object of the conspiracy, each shall be punished by imprisonment for any term of years or for life.

(d) Whoever attempts to violate subsection (a) shall be punished by imprisonment for not more than twenty years.

(e) If the victim of an offense under subsection (a) is an internationally protected person outside the United States, the United States may exercise jurisdiction over the offense if (1) the victim is a representative, officer, employee, or agent of the United States, (2) an offender is a national of the United States, or (3) an offender is afterwards found in the United States. As used in this subsection, the United States includes all areas under the jurisdiction of the United States including any of the places within the provisions of this subsection, the term �national of the United States� has the meaning prescribed in section 101(a)(22) of the Immigration and Nationality Act (8 U.S.C. 1101(a)(22)).

(f) In the course of enforcement of subsection (a)(4) and any other sections prohibiting a conspiracy or attempt to violate subsection (a)(4), the Attorney General may request assistance from any Federal, State, or local agency, including the Army, Navy, and Air Force, any statute, rule, or regulation to the contrary notwithstanding.

Dr. Manning Sloan made a referral to Forsyth Hospital to complete the treatment of blood clots in legs.

After beginning Heparis and Coumadin in the hospital and staff begins successful treatment of DVT. Dr. William Lawing makes referral to Bradford Village West Assisted Living Facility.

Working with hospital social worker-the doctor made referral to assisted living facility

A poorly run facility which I moved into in May, 2011 with referral from Forsyth Hospital. The Assisted living facility has a resident doctor who is canceling medical treatment by my outside Dr. at Prime Care.

The rooms and buildings are dirty; residents roam through other persons room without question... and apparently like to fist fight....

1. By written letter of instruction Dr. Tripp was replaced as treating physician 5-18-2011 and replaced with Prime Care of Greensboro, NC

2. Dr. Tripp ORDERED a chest x ray and EKG which the other hospitals had done within a few days of his request and I personally delivered the paper work to his staff. Appears to be an attempt to defraud Medicaid by duplicating unneeded test.

Dear Gloria:

I really enjoyed my brief stay at your assisted living facility and the excellent services and outstanding efforts of your staff on my behalf.

It appears your in house Dr. Tripp and my doctors at Prime Care had a difference of medical treatment and I was given to understand Dr. Tripp canceled medication for breathing problems prescribed by Prime Care. While I have an excellent existing communication and understanding with the doctors at Prime Care I have had problems getting an exact explanation of Dr. Tripp.s treatment plan and noted changes in medications without my approval or any explanation.

Today the visiting nurse who comes in daily to check my INR and Coumadin explained she had noted a 5mg increase in Coumadin level scheduled for Friday. She explained that since my current INR is therapeutic an increase in medication could easily be fatal and urged me to contact Dr. Tripp. I have no means of how to contract Dr. Tripp however tried and failed to have Prime Care take over as primary care physician because they do not accept Medicaid.

Under these circumstances I appreciate your concurring with my intention to move to another location out of state where I also have had Medicaid and already have a primary care physician.

Hospitalized at main hospital after referral from Klein Clinic for several days service and staff excellent.

I have been on medication Zantac for months and this medication is not recommended for persons being treated for blood clots. Clinic staff put me on prilosec

Assistant Director Bill Christian has made a referal to the Rescue Mission in Louisville, Kentucky as a possible excellent place for me to stay to complete my medical work.

Blood thinners create problems breathing at temperatures above 75 so I will try and spend the summer in the cooler temps of the Pacific NW
