Dear Cone Health:

Many thanks for the excellent services in received in the ER recently at Wesley Long Hospital and for the excellent and outstanding efforts of Nurse Case Manager Kimberly Gibbs RN (336-832-1298). In reviewing your bill and several services I would like to question if some elements of medicaid fraud might becoming apparent:

1. In 2011 in was seen by Dr. Fuller at the ER shortly after I was discharged from Wake Forest Hospital for treatment of blood clot. The XRAy tech at your ER asked why she was doing another sonar exam since the doctor could easily request the sonar exam from Wake Forest.

2. In March, 2011 I was admitted to Moses Cone for treatment of blood clot in legs and told I would be receiving heparin drip, coumadin� You can imagine my surprise when I woke up the next morning with a medical crew in my hospital room explaining they were there to prepare me for surgery. Nobody could explain who proposed the surgery or authorized it-so I left the hospital against doctors advice.

3. I explained my medical history to your Wesley Long ER staff and that the only effective treatment for the blood clots would involve several days of heparin to convert to coumadin. Instead of following my instructions the staff instituted a treatment of Lovenox and a successful treatment of the stalph infection.

4. A follow up with Dr. Kim Lykins at Moses Cone Urgent Health on 5-23-2012 has the doctor prescribing Aggronox even after I explained I would and could not take it at this time.

To avoid any future misunderstandings or questions I have enclosed an advanced patient directive as to what is and is not appropriate treatement based on 300 doctors efforts over the last 20 years.

I would recommend your medical staff refraim from giving the appearance of medicaid fraud by not duplicating expensive treats do by other medical authoritities; do not initiate treatement or surgery where the patient has advised your staff it is not appropriate; and many thanks for your efforts.

As you can see from the enclosure the Social Security Commissioner has sent word back that he will request a hearing pursuant to Title 42 USC 405(g)(H) Judicial Review at some time in the future in SSI claim 261-90-6357. The enclosed documents also suggest my doctors have explained I am at serious risk or stroke or other injury due to chronic blood clots. The enclosed materials are intended to provide your legal staff with adequate discovery documents to proceed if a hearing occurs and I am incapacitated at that time.

Your staff can see the several US Courts have defined black letter law and quite a few other items including 3rd party jonder; 3rd party tampering; �. have been moved to the several Committees of the US Congress. When the hearing goes to bar it may be possible to exclude the unauthorized matters so as not to burden the Court.

1. Title 28 USC 1331 Federal Question provides a US District Court jurisdiction for actions at bar where the Constitutionality of Acts of Congress have been brought into question.

The copy of the Notice of Withdraw in USDC was stamped recieved and then mailed back to me by the clerk of the United States District Court, Middle District of North Carolina. The envelope has been torn open and sloppily taped back together. As this link suggest the Honorable FBI Director Robert Mueller III has to appear before Congress and apologise for Agents engaged in unlawful conduct...I would suggesting the next fed caught intercepting any type of USDC document or ORDER be transfered to Antarticia to shovel penguin poop.

Lacking any apparent lawful jurisdictional authority to engage such an action the principals should be considered "terrorist" under UN Security Council Protocols and subject to termination on sight.

NOW COMES THE Plaintiff Pro Se Chris Walters before the Honorable United States District Court for the Middle District of Carolina to wit:

1. Exhibit A: The Plaintiff Chris Walters is currently suffering from chronic medical condition Deep Vein Thrombosis and Exhibit B unable to secure a Medicaid Card to secure adequate medical treatment and medications.

2. Chris Walters will have to move to another venue to secure medical treatment or face the prospect winning the Court case and dying or suffering a disabling injury.

3. The Commissioner of Social Security has been afforded an opportunity to see if extensive 3rd tampering has occurred in the SSI claim at bar and has other means of redressing the problem if need be.

4. Members of the US Senate Committee on the Judiciary can if they so desire examine the evidence as evaluate the operation of the Social Security program to see if improvements can be made in the law.

5. As Chris Walters will have to move to another region of the United States he will be unable to Support a litigation action here in USDC, MDNC.

Given the benefits of Medicaid Chris Walters could have been placed in a skilled nursing facility or assisted living facility and possibly had one of the numerous medical hospitals or 3 teaching colleges in the area complete the recommended hip surgery; surgery on both knees; gallbladder surgery; treatment for chronic DVT; placed Chris Walters on CPAP machine again�.. Unfortunately Chris Walters housing and health options are out of time and a move to another venue seems the best option.

In the future if members of the US Senate Judiciary Committee here Congressional members from North Carolina complaining about rising costs of Health Care, Chris Walters would suggest they be given a copy of this brief and asked to get a new story.

2. 18 USC � 1091 - Genocide (a) Basic Offense.� Whoever, whether in time of peace or in time of war and with the specific intent to destroy, in whole or in substantial part, a national, ethnic, racial, or religious group as such�

(1)kills members of that group;

(2)causes serious bodily injury to members of that group;

(3)causes the permanent impairment of the mental faculties of members of the group through drugs, torture, or similar techniques;

(4)subjects the group to conditions of life that are intended to cause the physical destruction of the group in whole or in part;

(5)imposes measures intended to prevent births within the group; or

(6)transfers by force children of the group to another group; shall be punished as provided in subsection (b).

(b) Punishment for Basic Offense.� The punishment for an offense under subsection (a) is� (1)in the case of an offense under subsection (a)(1), where death results, by death or imprisonment for life and a fine of not more than $1,000,000, or both; and (2)a fine of not more than $1,000,000 or imprisonment for not more than twenty years, or both, in any other case.

(c) Incitement Offense.� Whoever directly and publicly incites another to violate subsection (a) shall be fined not more than $500,000 or imprisoned not more than five years, or both.

(d) Attempt and Conspiracy.� Any person who attempts or conspires to commit an offense under this section shall be punished in the same manner as a person who completes the offense.

(e) Jurisdiction.� There is jurisdiction over the offenses described in subsections (a), (c), and (d) if� (1)the offense is committed in whole or in part within the United States; or (2)regardless of where the offense is committed, the alleged offender is� (A)a national of the United States (as that term is defined in section 101 of the Immigration and Nationality Act (8 U.S.C. 1101));....
