I would suggest contacting the Charles Drew Center for medical care. Also, the Elon BioBus runs routes throughout Burlington and Elon, but I am not sure how much they charge.

The Social Security Commissioner has gone to several US Courts and Quashed or prohibited any discussions which might involve most anything we would have of merit to discuss with her approval pursuant to Title 42 USC 405(g)(h) Judicial Review having to first be approved in advance by a Federal Judge:

In 2004 I was co defendant Bradshaw v Walters, Hooker Case 04CVS2674 in a NC State District Court and as the Court ruled no discussion of anything related to social security is appropriate without the explicit permission of the Commissioner of Social Security. That the primary matters we would discuss or case management are closed is unfortunate; but I can explain I am seeking admission to assisted living and giving the problems I am having finding a doctor am sure I haven’t got a chance to get where I need to be.

I wanted to commend your organization for maintaining the CHIN network for homeless case management and data tracking to improve services to the homeless. As you are aware most of the homeless shelters are requiring clients to subscribe to CHIN services and engaged in case management. Unfortunately as you can see from the enclosures the Social Security Administration has secured several ORDERS from US Courts which restrict discussions without the Commissioner filing a brief in a US District Court as required by Title 42 USC 405(g)(J) Judicial Review. North Carolina Department of Health and Human Services has also restricted information related to Social Security in Bradshaw vs Walters, Hooker State District Court at Winston Salem, NC Case 04CVS2674.

The appended letter from Donna Hodges of Allied Area Churches of Alamance County shows I am being discharged as a client on 5/9/2013 instead of 6/9/2013 because of a failure to attend case management. The case manager is probably unaware that such discussions are blocked by existing ORDERS of Courts The Social Security Administration has recently been in USDC and gone to quite a bit of trouble to get my Medicaid benefits turned on in North Carolina and I am working actively with Alamance County Department of Social Services to get the special Medicaid to get into assisted living and the name of the contact person is:

1. Program Requirements: From the time I move into town it takes Social Security and Medical from 3-5 weeks to turn on my benefits in the state I proposed to reside in. To make the plan work I have to have a primary care physician who is willing to Accept Medicaid. Finally the doctor has to sign an Order for admission. The process can easily require 2 months.

2. Plan A Hickory NC as you can see from the enclosed letter I was in Hickory, NC got a doctor who is reluctant to accept Medicaid, consulted with Hickory Manor whose replied seemed very positive. It should have take the doctor 2 days to get me into the facility and a month latter and many stories later I am getting no replies to my emails inquiries.

3. Plan B Virginia see if an option exist to move here and get into assisted living.

4. Plan C If no other options available survive until August 1, 2013 get next SSI check and move to Western US where I can simply rent a room from shelter.

You can come to our community shelter to sign up for a bed on Mondays or Thursdays. Our shelter is located at 410 Liberty Street in Durham, downtown, across from the county library. You can line up outside the shelter at 9:15am and we'll start taking names at 10am. If you sign up on a Monday you have a bed reserved until the following Monday, if you come in every night. If you sign up on Thursday you have a bed reserved until the following Monday when everything starts over. As long as you are able to handle your own basic daily living functions without assistance and you come to the shelter to sign up you'll be able to get a bed in our shelter.

We also have a program for clients with disabilities called Journey Outreach. Chris,

We have a dorm for 65 and older, or unable to get into top bunks, but it is not a recovery bed. The time table of the mission is still enforced, you cannot be in bed from roughly 5:00am to until after chapel at roughly 8:00pm. You need to be able to care your needs including hygiene. We serve, 3 hot meals, showers daily, beds which are first come first serve, with some modification to provide accommodation documented special needs for a fixed period of time on a case by case basis. There are 350 -500 men here nightly and every meal.

Campus for Human Development (note below) is another service ministry, close to the Rescue Mission that has respite beds for hospital recovery that, I think, are available from 1 wk to 30 days ( I am not sure). After that you could make plans to be at the mission. Room at the Inn except for special veterans, A&D rehab, and some mental illness beds is not a permanent rooming situation.

From Hickory Record HICKORY, N.C. — A ribbon cutting was held July 12 to officially open The Brigadiers Charles and Evelyn Sams Mens Transitional Housing. The transitional housing building is located on The Salvation Army campus on Lenoir-Rhyne Boulevard. The event was attended by Salvation Army officials, advisory board members, members of the Charles and Evelyn Sams family, Hickory Mayor Rudy Wright, representatives from The Unifour Foundation and the Catawba Valley Association of Realtors, The Catawba Valley Quilters Guild and volunteers who worked on the housing facility. Those organizations and individuals all helped make the housing facility possible by donating money and time.

new transitional housing facility in Hickory
The transitional housing facility will serve 11 men who are employed but homeless. The design of the program is to equip the men with the tools necessary for them to become self-sufficient and break the cycle of homelessness. Classes will include budget counseling, substance abuse classes, job training and GED classes. The client can stay in the program for up to two years by which time they will have saved enough money for a deposit on a home and have a financial cushion.

The dedication started with a welcome and prayer from Capt. Michael Harris. Introductions were made and Eddie Sams, son of Charles and Evelyn Sams, unveiled the sign on the building and made a few comments. Lt. Col. Willis Howell, divisional commander, delivered a message about The Salvation Army’s holistic approach to salvation. Capt. Michael Harris, the Hickory Corps officer, said, “This has been a vision of mine since Capt. Christine and I were assigned to this command three years ago. We now have the means to not just provided emergency shelter for the homeless but we can give the men a hand up to get back on their feet and the opportunity to establish housing of their own.”

“There are more than 250 homeless people in Catawba County and this community has responded generously to meet the needs of the homeless, but the great thing about this transitional housing is that it is not enabling people to live this lifestyle but helping then become self-sufficient,” Wright said. Steve Aaron, resource development director, expressed appreciation to The Unifour Foundation, whose $11,000 grant provided the new showers that were constructed, and to The Catawba Valley Association of Realtors who will donate $14,000 from their Golf for Charity Tournament to pay for the new sprinkler system. A special acknowledgment was given to the Catawba Valley Quilters Guild, whose members stitched new red, white and blue quilts for each bed, and to NCFI Polyurethane, which donated mattresses for the facility. The Sams family was recognized for their gifts in honor of their parents, Charles and Evelyn Sams that helped make this facility possible. Board member Beth Whicker and her husband Les were recognized for their work to furnish and organize the building. After the ribbon cutting, an open house was held, with refreshments provided by Chick-Fil-A.
