Dear Case Worker Jo Ann May:

I spoke to the Lovenox manufacturer on the phone yesterday who explained his product is not designed for long term indiscriminate use without proper blood testing and monitoring. The representative explained that Lovenox is designed to be used for several weeks in conjunction with Coumadin.

I have already been on Lovenox for 6 months and Providence Hospital prescription to throw another $12,000 or two month supply of Lovenox at the DVT seems inappropriate. The Hospital reported in April, 2010 when I was in patient that they could not get the Coumadin to reach the therapeutic level of 2-3. I of course will not take Coumadin on an outpatient basis if the hospital can’t balance it.

I spoke to the Charge Nurse at Providence Hospital who had no other suggestions for treatment.

Because recent experiences have shown that homeless shelters are not a suitable environment to base serious medical work I have created this new patient medical directive concerning DVT. I will delay treatment until I can find a doctor/hospital which will do the medical work in house such as:

1. Attempt Coumadin in a hospital/nursing home setting in event internal bleed occurs.

2. Lovenox manufacturer said their product for short period of time.

If a hospital/nursing home can’t get meds to balance I might try something else.

Many thanks for your expert assistance as primary care physician-an unforeseen problem has arisen which causes me to request cancelation of my medical appointments/referrals.

Medicaid has paid for my last visit to St. Alphonsus Hospital however is declining to pay the medical bills of Dr. Samuel S Gibson MD (208-367-6030) and Dr. David Schmitz Md (208-367-6030) and created an unpaid bill of about $3,500. At this time we offer 30 consecutive nights twice a year. These two periods must be separated by one night. If this would suit your needs bed space is almost always available. Subject RE: Question use of toxic chemicals in habitated building

If I understand what you are saying, staff members are disinfecting people by directly spraying bleach onto people? If so, are they using bleach in a commercial concentration or following recommended practices of diluting the bleach for disinfection to a 10:1 ratio with water and allowing a twenty minute soak time. In either case, this practice was generally developed by the military but is not recommended by the CDC for use on persons. Only on environmental surfaces. I will forward this email to the Environmental health authority here in town and get her opinion. Also, allowing chlorine or chloramine gas concentrations to accumulate in enclosed, unventilated work or living areas without directly tracking the concentration is NEVER a good idea and in a workplace setting it is technically a violation of OSHA law. As the enclosed materials indicate I have asked several Congressional Committees to review aspects of my SSI claim as it relates to 3rd party tampering primarily by various concerns associated with the US Department of HUD.

When a nonprofit agency indicates they do no accommodate persons with disabilities or ends a course of medical treatment being paid for by Medicaid the medical payment system is losing an estimated $50,000 per event.

As your office recalls the several federal courts have upheld the rights of the Commissioner to a hearing pursuant to Title 42 USC 405(g)(H) Judicial Review. I am unaware of any 3rd party right interplead; join, or otherwise litigate in the matters.

Fairly nice facility but some distance from town. This shelter might be useful place to try and get medical work done if I can find a doctor.

LONGVIEW CBS 19 (KYTX) -- There's more help coming for the hungry and homeless in Tyler, and it's coming from Longview. The Hiway 80 Rescue Mission is trying to fill a void in Tyler's homeless services.

Hiway 80 tells CBS 19 the East Texas Rescue Mission's decision to close down in Tyler left a huge gap that a lot of people are falling into. Now they're taking the first small steps toward bringing services to the Rose City that will get the homeless back on their feet. After a month in the hospital, James Green had nowhere to go. After turning to Hiway 80, he's come to believe it represents a new beginning.

"The Salvation Army down in Tyler said I could only stay for three days," Green said. "So the hospital gave me money for a cab to come here." He wasn't always a man with nowhere to go. In fact, Green lived all over the U.S. until his health took a hit, living homeless in Tyler. "We're sleeping about 170 per night in this facility and at different times up to 25 percent are from Tyler," Hiway 80's Executive Director Eric Burger said.

Those numbers have climbed ever since the East Texas Rescue Mission in Tyler closed its doors in 2010. "I began volunteering once a month [at ETRM] as the chaplain, doing the service for them," Hiway 80 case worker Antonio Christian said. Christian's volunteer experience with the East Texas Rescue Mission led him to Hiway 80 and Hiway 80 came to him when they realized it was time to expand into Tyler. Now he spends his days networking one-one-one with Tyler's homeless and with other non-profits that could help them.

"They've got a clinic called Bethesda Clinic in Tyler, where I went and talked to one of the volunteer coordinators," he said.

It's a grass-roots effort that's a long way from putting new beds inside a new shelter. For the time being Hiway 80 is working on little things like doubling the three nights a week that local churches offer meals to the homeless. "Let this be a transition from one point in your life to another point in your life. Don't let this be all your life," Christian said. "I'm very thankful for it," Green said. He plans to leave later this month and head to Wyoming. "It's just a true blessing. Only God can do that for you."

A group of Tylerites calling themselves the Committee for a Rescue Mission in Tyler are pitching in to help make Hiway 80's expansion a reality. They tell CBS 19 they're working toward a more complete strategy for helping Tyler's homeless and hungry.

Dear Good Shepherd Medical Center:

I wanted to thank your staff for their excellent work during my recent hospitalization and the work of Dr. Wen the Internal Medicine Specialist in treating the acute/chronic DVT. As great fortune would have it I passed 10 liters of water in 24 hours and at the same time lost 15 pounds.

The attending physician has asked about my having to move from place to place which creates difficulties in treatment. If you visit this website your staff can see an operational audit of the social security process submitted to Congress:

Megabytes of data show the homeless shelter system operates a travel agency; and you can easily see many instances of shelters blocking medical treatment by stating they do not provide extensions; or for instance the Tyler Salvation Army does not accommodate disabled persons, the local mission would not accommodate Dr. Payne and dispatched his request for bed rest as lower than poop on a dog's hind leg. Unfortunately neither your staff; nor myself; or even the government can tell a nonprofit agency how to operate.

Because of this problem in the past several of my primary care physicians have recommended I be placed in a nursing home for a short period of time. Since Oregon went broke about 20 years ago; Texas is projecting a $19 Billion budget short fall I am moving to North Carolina which hopefully might be have the Medicaid funds to move forward

Moving Forward With Recommendations
for Nursing Home
Dr. Ruth Thomas of Salem, Oregon and Dr. Dr. Katherine Weidner of Medford Oregon have recommended stay in a nursing home to accomplish:

1. Complete conversion to coumadin with daily monitoring of INR because of problems in getting INR to balance.

2. Monitor chronic bleeding-if bleeding occurs what better place to be than in a hospital/nursing home.

3. Dr. state I require gall bladder surgery for removal of gallstones.

4. Orthopedic surgeon can evaluate problem with left his dislocation.

Because of problems with tampering by social services I have created an advanced medical directive to block treatment of DVT on out patient basis. I believe it will be cheaper in longrun to treat conditions rather than have endless series of visits to ER.
