Dr Ghalyoum has prescribed medications Lisinopril and Metoprolol but noted the medications would have to be balanced later

Social Worker is attemping to make referral to nursing home however spends more time trying to scold me for arriving in Texarkana than help me get treatment for:

1. Probable blood clots in leg or cellulitis

2. Declined to provide antibiotic for infections

3. Failed to provide CPAP machine for treatment for partially collapsed right lung

4. The Social worker state that if the nursing home referral does not work out no other agency in this region would help him any further

As the admitting Emergency Room Physician explained lassix can become ineffective however the cause of problem was reduced heart fraction ejection 45% and need to change diuretic

Water building up is inevitable with Cardiac Fraction Rate 45% and extensive kidney damage caused by abuse of Lassix as medication/cure all

Diagnosis stuck in fruitless efforts to administer Lassix which is no longer effective orally and throws off medical work to physician assistant who is not qualified to identify serious medical issues.

The Facility Manager agreed to address issues to improve services some problems include

1. Failure to remove feces from room 2 days in row staff is often slow to respond to request for assistance.

2. Possible roaches or other bugs in room

3. Lack of CPAP or oxygen machines for treatment after 4 days

4. A box fan brought into room to substitute for oxygen machine is full of dirt

5. The Nurse Home management finally agreed to accept patients medical records and Advanced Medical Directive on flash drive

6. I haven't been permitted to shower for up to 5 days

7. Have been in isolation for well beyound 10 days and the person in charge of room assignments states they have not prepared a non isolation room for me.

8. I am pooping yellow and loose stools for about 2 weeks and may have problems with water or food (Had similar problems in San Antonio, TX)

Meagan NP 8-08-2022 has not schedule an appointment with Urology Surgeon as she suggested last week; or caridac specialist; and suggested she wants to substitute my medical treatment with physical therapy.

After numerous interviews with physical therapy staff; nursing staff...and everyone else I have finally gotten through that no PT or other activity will occur until my kidney problem is treated

1. The animus of the possible violation of law is that the nursing home and medial staff overstepped patient rights to require needed medical treatment base on the desire of the agencies involved to defraud Medicare.

4. On Sunday August 14th, 2022 Chris Walters asked for medical transport to the hospital for evaluation of possible gallstone issues was denied by the nursing staff with the statement that Nurse Practioner Megan would be in on Monday

6. It appears that the nursing home and medical staff are unaware that after a criminal complaint is filed with a federal agency the parties are not permitted to communicate further as all discussions are reserved to prosecuting authorities.

The popular television on the Animal Planet concerns care and treatment of animals at the San Diego and Bronx Zoos. No amount of expense; care and treatment is spared in providing for the various species in the zoo and it might be asked if the animals could be returned to their native countries and replaced with patients in long term care facilities? In the last 4 years numerous medical providers have been unable to remove a cancer in Chris Walters left kidney; complete diagnosis and treatment of congestive heart failure; remove bad teeth or address many other medical concerns while long term care home receive a reported $8700 a month billed to the government. The question submitted to the United States Supreme Court in a Petition For Writ of Certiotari DOCKET 19-7809 asks if ongoing challenges to the Rule of Law would eventually lead to anarchy or civil war the known facts being cited on this Congressiona Archive:

In more plainer the facility administrator Shelia Milam placed an application for Arkansa Medicaid and explained the home is required to garnish my SSI retirement which is prohibited by federal law and Court ORDERS. By September 1, 2022 I have to decide to forego treatment for kidney cancer to move to another state which does not garnish my $659.00 retirement check; move into a homeless shelter; or out of Arkansas into hospice.

Dear Administraor Shelia Miliam:

Many thanks for your excellent work and issues and request to access SSA retirement funds and access Direct Express Card data on or about 9/1/2022. As you can see the Social Security Administration has gone to court to prevent any seizure of funds; changes in SSA status; unauthorized changes in medical treatment....

The enclosures marked US Supreme Court show the questions and issues have been removed as national policy issues reserved to Congress which I imagine neither myself or the nursing home want to address and the people who run the country can figure it out.

I anticipate that this event will require me to leave the nursing home 9/1/2022 as failing to meet your facilities requirements and I might ask for a meeting this week to discuss possible exit ideas which might include:

1. Moving into hospice in or near Longview or Tyler Texas.

2. Moving into homeless shelter possibly in Longview Texas a few of which are set up to accomodate persons with medical issues.

3. Setting up Amtrak ticket to cross the US and return to western United States.

4. Other options I might not be aware of.

As chance and circumstance had it I found your email online a few minutes ago and this seems to be the best way to transfer files. I also wanted to commend you on your hard work and efforts in working with the endless problems which occur in the normal operation of nursing homes...

During a meeting with Administrator:-Sheila Milam and "Jeff" and investigator from the State we reviews concerns:

1. It is not clear Dr Clay Ferguson has ever received or had a chance to review Chris Walters medical records provided to nursing home on flash drive.

2. The medical staff is unable or unwilling to adhere to my Advanced Medical Directive

3. As the nursing home cannot garnish or received my SSI retirement check my stay must end on 9/2/2022

The nursing home will have a van drive me to Longview Texas on or about 8/27/2022 as I move on to explore other options the several days later changed their mind

The manager Shelia Milam states the facility receives $6,000 a month for maintence of each patient or $72,000 a year not including other medical costs.

Deprivation of rights under color of law Whoever, under color of any law, statute, ordinance, regulation, or custom, willfully subjects any person in any State, Territory, Commonwealth, Possession, or District to the deprivation of any rights, privileges, or immunities secured or protected by the Constitution or laws of the United States, or to different punishments, pains, or penalties, on account of such person being an alien, or by reason of his color, or race, than are prescribed for the punishment of citizens, shall be fined under this title or imprisoned not more than one year, or both; and if bodily injury results from the acts committed in violation of this section or if such acts include the use, attempted use, or threatened use of a dangerous weapon, explosives, or fire, shall be fined under this title or imprisoned not more than ten years, or both; and if death results from the acts committed in violation of this section or if such acts include kidnapping or an attempt to kidnap, aggravated sexual abuse, or an attempt to commit aggravated sexual abuse, or an attempt to kill, shall be fined under this title, or imprisoned for any term of years or for life, or both, or may be sentenced to death.
